Scoring Scale
Each question will be answered on a scale of 1 to 5, where:
1 = Strongly Disagree
2 = Disagree
3 = Neutral
4 = Agree
5 = Strongly Agree

This comprehensive assessment contains 95 questions and will take 15 - 30 minutes to complete.

Receiving Your Report
You will received a detailed 16 - 17 page report. Based on your responses, this report will outline key issues your organization faces in each of the six areas.
The report will include specific challenges, faced by both business leaders and employees, as a result of the dysfunction. It will also provide 5-10 actionable recommendations for each dysfunction area to help mitigate these issues.
